In short
How should churches be led? In the New Testament, ‘bishop’ (overseer) and ’elder’ are two names for one office, and each church had several. By about AD 110 Ignatius describes one bishop over the elders and deacons, and by 200 this was everywhere, with bishops in succession from the apostles. Jerome later explained that one elder came to preside ’to remedy schism’. Episcopal churches follow the fathers’ pattern; Presbyterian and Baptist churches follow the New Testament wording.

Overall verdict
Ordained ministry in succession from the apostles is Era I; the single bishop is near-universal by Era II. Scripture, though, uses ‘bishop’ and ’elder’ for one office, as Jerome later says. Episcopal traditions match the fathers; presbyterian and congregational ones match the NT terms more closely.
